This combination makes the diagnosis of UTI one of the leading drivers of antimicrobial use, which is of particular concern with rates of antimicrobial resistance in Gram-negative organisms rising out of proportion to new drug development. Multiple syndromes fall under the umbrella of “UTI,” ranging from cystitis, to the more severe pyelonephritis, to the potentially life-threatening urosepsis. Recognizing the differences in diagnosis and treatment, rather than having a one-size-fits-all approach to every UTI, helps guide clinicians to appropriate antimicrobial choices and management decisions. In addition to recognizing different UTI syndromes, being aware of complicating factors is important in the management of UTI. Complicating factors are those that increase the risk of UTI, make eradication of the infection more difficult, broaden the spectrum of causative organisms, or increase the risk of UTI recurrence. Even in the era of increasing antimicrobial resistance, treatment for most cases of UTI can be completed with oral therapy, although parenteral treatment should be used initially for most cases of severe infection such as pyelonephritis, and certainly for urosepsis. Having knowledge of and access to local antimicrobial susceptibility patterns are vital to selecting appropriate empiric therapy for all UTI syndromes but are especially important when dealing with acutely ill, hospitalized patients." @default.
