METHODS: In the whole cohort (WC) series,pts with RENAL<6 were excluded. Selectively collected patients with Deep masses (Deep) had item “E”=3 of RENAL. K-M analysis was performed to assess the risk of developing CKD stage 3b during the follow-up between the two groups. A sub-group analysis was then performed, after RENAL score stratification. UV and MV analysis were performed to identify predictors of severe renal function deterioration after surgery. RESULTS: Overall, 1314 pts were included, 341 Deep and 973 WC. The probability of developing a stage ≥3b CKD was significantly higher (p=0.001) in the Deep cohort (1-, 3- and 5-year risk 3.9%, 12.1% and 17.7% vs 1.6%, 3.3% and 6.3%). In the RENAL <9 sub-group analysis, K-M curve did not show any statistically significant differences (p=0.45). In the RENAL ≥ 9 sub-group analysis, Deep group displayed a statistically significant higher risk of severe renal function deterioration over time (1-, 3- and 5-yr risk 4.4%, 13.4% and 13.4% vs 2.3%, 4.8% and 7.4%, respectively:p=0.05). At MV Cox regression analysis, Deep status and baseline eGFR were independent predictors of worse renal functional outcomes. For each increasing ml/min of baseline eGFR, the risk of developing a stage ≥3b CKD was reduced by 2% (HR 0.98, 95% CI 0.96–0.99; p=0.02); the Deep status was associated with a 1.99-fold increased risk of developing a stage ≥3b CKD (HR 1.99, 95% CI 1.09–3.64; p=0.02). CONCLUSIONS: According to their technical complexity, deep renal masses have to be considered as a separate category of tumors. Regardless of RENAL score, they show a higher rate of severe renal function deterioration over time, being inevitably exposed to a 2-fold increased risk of developing a severe CKD during follow-up.
