- W4207048462 abstract "We thank Dr. Ramsay and Dr. Stanich for raising this hot topic in gastrointestinal endoscopy. Cold snare polypectomy (CSP) has become an increasingly accepted technique for polyp excision, being already the gold standard for smaller polyps. As we have pointed out in our review,1Rutter M.D. Jover R. Personalizing polypectomy techniques based on polyp characteristics.Clin Gastroenterol Hepatol. 2020; 18: 2859-2867Abstract Full Text Full Text PDF PubMed Scopus (2) Google Scholar CSP has demonstrated superiority to forceps biopsy excision in polyps between 3 and 5 mm and also with similar efficiency than hot snare in polyps up to 10 mm, but with fewer complications. However, evidence for the role of CSP as the preferred option for polyps larger than 10 mm is not yet so robust. Use of hot snare polypectomy for resection of 10- to 19-mm polyps has been the standard of care for a long time. Although evidence supporting the use of CSP in these polyps is increasing (particularly for sessile serrated lesions), it currently only comes from observational studies as cited by Ramsay and Stanich. In our review, we encourage the use of CSP in general and describe the potential use of CSP for nonpedunculated polyps of 10–19 mm, outlining its potential advantages, especially where the risk of post-procedure diathermy-related complication is higher (eg, in the cecum). However, in our opinion, there is not yet sufficient evidence to recommend CSP as the preferred option for these polyps. Randomized controlled trials are ongoing (NCT04636619) and we await results from these studies with interest." @default.
