In frühen Studien lag die Gesamtansprechrate auf eine Ovarektomie für Patientinnen mit Mammakarzinom bei ca. 30% [5], Dies entspricht den heutigen Ansprechraten auf eine endokrine Therapie in unselektionierten Patientinnenkollektiven. Ähnliche Ansprechraten wurden sowohl für die Adrenalektomie als auch für die Hypophysektomie erzielt, im klinischen Alltag spielt heute jedoch nur noch die Ovarektomie eine gewisse Rolle. Trotz der eindeutig positiven Effekte einer endokrinen Therapie, blieb der Grund für das heterogene Ansprechen der Mammakarzinome auf diese Therapieform ungeklärt, bis in den 60er Jahren der Östrogenrezeptor von Toft, Gorski [68] und Jensen [42] identifiziert wurde. Mittlerweile ist bekannt, dass die Wirkung von Östrogen über den Östrogenrezeptor (ER), ein Mitglied der Familie der nuklearen Rezeptoren, vermittelt wird. In dem Kapitel über Steroidhormonrezeptoren wurden die molekularen Mechanismen beschrieben, über die der Östrogenrezeptor die Transkription von Zielgenen aktiviert. Die Bestimmung der Östrogenrezeptorexpression in primären Mammakarzinomen hat eine Vorhersage bezüglich des Ansprechens auf eine endokrine Therapie ermöglicht. Heute sind Antiöstrogene zentraler Bestandteil der Therapie von Mammakarzinomen aller Stadien. Von den fortgeschrittenen östrogenrezeptorpositiven Mammakarzinomen sprechen ca. 2/3 auf eine Therapie mit Antiöstrogenen an [31]. Die Mehrzahl der östrogenrezeptorpositiven Tumoren werden, auch wenn sie initial auf eine antiöstrogene Therapie angesprochen haben, résistent gegen diese Therapie [61]." @default.
