- W4211194635 abstract "For unknown reasons, there has been a notable reluctance on the part of pediatric cardiologists to recognize the presence of significant ventricular arrhythmias in childhood. This attitude was no doubt historically influenced by the relative rarity of sudden death among children, as well as the knowledge that ventricular tachycardia was most often related to the presence of coronary artery disease, certainly not a pediatric affliction. Although it is well known that normal children will occasionally experience ventricular ectopics, the assumption that these are generally benign is so deeply impressed that the notion was not confirmed until 1978 [1]. The possibility of ventricular tachycardia in children was rarely considered, though cardiologists were relieved to learn (in 1979) that this arrhythmia could also be benign among children [2]. To an unfortunate degree, however, these long-held assumptions have reflected wishful thinking rather than documented fact. Syncopal children have sometimes spent years receiving anticonvulsant drugs because cardiac arrhythmias were not considered in the differential diagnosis. Sudden deaths have been ascribed to obscure coronary malformations or seizure disorders before an arrhythmia was postulated. This attitude is widespread. In preparing this chapter, for example, a search of the computerized cardiology records of the Toronto Hospital for Sick Children revealed only three properly coded cases of ventricular tachycardia before 1981, out of more than 30,000 cardiac patients seen up to that point. Cardiologists apparently could not, or would not, bring themselves to make such a diagnosis in a child. Even today it is common to see knowledgeable physicians invent convoluted explanations involving supraventricular aberrancy or odd-looking “nodal” beats to explain obvious cases of ventricular tachycardia." @default.
