- W4213308385 abstract "Pulmonary hypertension (PH) is an under recognized driver of cardiovascular morbidity and mortality in patients with chronic kidney disease (CKD). It's prevalence has been reported as 21%-41% in patients with CKD and upto 60% in those on maintenance haemodialysis (MHD). This was a cross-sectional study on 143 consecutive adult hemodialysis patients from a tertiary care centre in Kolkata, India. Clinical and laboratory data of the patients were recorded. These patients had their systolic pulmonary artery pressure (SPAP) measured by echocardiography and right ventricular (RV) function assessed by tricuspid annular plane systolic excursion (TAPSE). PH was defined as SPAP > 35mm Hg, and RV dysfunction as TAPSE <1.7. PH was considered to be post-capillary in type, if E/e’ > 15. Mean age of patients was 62.71±13.17 years, of which 61.5% were males. The prevalence of PH was found to be 46.2% (66/143). Mean SPAP was 34.45 ±15.99. Post-capillary PH, was identified in 72.7% (p = <0.0001). The predominant cause of PH was thus left heart disease. (WHO group 2). All the patients with PH had diastolic dysfunction (DD), of which 41.7% had heart failure with reduced ejection fraction (HFrEF). Ischaemic heart disease was found in 47% (p=0.001) and a dilated left atrium in 50%. Amongst lung causes (WHO group 3), 6.1% of PH patients had obstructive sleep apnea and 12 % had chronic obstructive airway disease. Pulmonary embolism (WHO group 4) was demonstrated in 2 patients. Chronic liver disease (WHO group 1) was found in 9.1%. However since all the patients had DD each patient has to be categorized as WHO Group 5. There was evidence of subclinical right ventricular (RV) dysfunction in 6.3% patients; of which two thirds had PH. Pericardial effusion was present in 29% of PH patients and seemed to be a marker of severity. It was present in all the patients with RV dysfunction. No association was found with type of vascular access or duration on HD. Pulmonary hypertension is present in nearly half of the patients on haemodialysis in India. The predominant cause was found to be due to left heart disease. Diastolic dysfunction was present in all the patients with PH and nearly half of them had HFrEF. RV dysfunction was present in a small fraction, two third of whom had PH. It is therefore imperative to look for causative factors of PH and try to prevent them in an endeavor to improve the cardiovascular morbidity and mortality of patients with CKD on HD." @default.
