- W4214540107 abstract "Background: The rate of inpatient mastectomies remains high despite multiple studies reported that outpatient mastectomies do not compromise quality of patient care. Major advantage of outpatient treatment is the increase of efficient and effective care. An outpatient mastectomy program was developed and gradually implemented in 2019; the entire surgical-anaesthetic team was included. Main objective of this study was to evaluate feasibility of outpatient mastectomy. Facilitators and barriers were identified in order to improve compliance. Materials and Methods: Implementation was evaluated in a retrospective study. All patients who underwent a mastectomy between January 2019 and January 2021 were included. Primary outcome was the rate of outpatient treatment. Secondary outcomes were differences between inpatient and outpatient mastectomies regarding unplanned return to care, readmission and reoperation and the reasons therefore. Multivariable logistic regression analyses were performed to identify facilitators and barriers for outpatient mastectomy. Results: (Preview of results): A total of 166 patients were enrolled in the study: 50% (n=83) were outpatient mastectomies versus 50% (n=83) inpatient mastectomies. A steady rise in outpatient mastectomies was observed in 2019. The second quarter of 2020, coinciding with the first COVID-19 wave, showed a peak in outpatient mastectomies. The sole significant barrier to outpatient mastectomy care proved to be an immediate breast reconstruction. Unplanned return to care was observed in 18% of the outpatient cohort versus 28% of the inpatient mastectomies (P=0.13) for similar reasons, such as clinical significant seroma and wound concerns. There were no re-admissions nor re-operations in either cohort. Conclusions: Outpatient mastectomy is shown to be feasible and safe. A steady increase in outpatient mastectomies was observed over two years. Unplanned return to care did not differ significantly between the outpatient and inpatient cohort.
