- W4214867180 abstract "Since the 1970s, randomized controlled trials (RCTs) have been the cornerstone of major therapeutic advances in hematologic cancer. The field has a long legacy of seminal trials that have led to remarkable improvements in outcome for patients with childhood acute lymphoblastic leukemia, acute promyelocytic leukemia, and Hodgkin lymphoma. Historically, these trials have been conducted by academic cooperative groups in an iterative fashion; each successive RCT is designed to build upon the results of the last. Core principles in this evolution of knowledge have been the use of randomized study design to identify meaningful gains in overall survival and quality of life. Recent work has shown worrisome trends with regards to these central tenets in contemporary RCTs of solid tumors [ [1] Del Paggio J.C. Berry J.S. Hopman W.M. Eisenhauer E.A. Prasad V. Results of the accompanying article by Wesson et al. should serve as an alarm bell; major problems also exist in clinical trials of hematologic cancer [ [2] Wesson W. Galate V.L. Sborov D.W. Characteristics of clinical trials for haematological malignancies from 2015 to 2020: A systematic reviewEuropean Journal of CancerVol. 167PreviewAs the landscape of haematological malignancies dramatically changes due to diagnostic and therapeutic advances, it is important to evaluate trends in clinical trial designs. The objective of our study was to describe the design of clinical trials for five common haematological malignancies with respect to randomisation and end-points. We also aimed to assess trends over time and examine the relationships of funding source and country of origin to proportions of randomisation and utilisation of clinical end-points.
