Opioids activate the endogenous opioid receptors in the body. Opioids vary in their pharmacokinetics, that is, absorption, distribution, metabolism and elimination from the body. Pharmacogenomic testing is emerging to provide additional information to guide clinicians in their selection of opioids for patients. Proper patient selection is critical and requires a thorough evaluation and a comprehensive risk–benefit assessment. In cases of opioid-induced hyperalgesia, the pain intensity might be reduced with opioid tapering. Monitoring of patients using opioids as part of their management for chronic non-cancer pain should be an inter professional responsibility. Management of opioid use during pregnancy must be done with clinical expertise and caution, since abrupt discontinuation of opioids leading to opioid withdrawal has been associated with miscarriage and premature labor." @default.
