- W4220947468 abstract "Both decreased and increased cell-free (cf) DNA fetal fractions are associated with adverse obstetric outcomes in singleton pregnancies. The aim of this study was to evaluate the association between cfDNA fetal fraction and adverse obstetric outcomes in twin pregnancies in a large geographically diverse population. This is a planned secondary analysis of data from a retrospective cohort study aimed to evaluate cfDNA screening in twins seen at 17 centers between 12/2011 and 2/2020. CfDNA screening was performed by one clinical laboratory using a massively parallel sequencing approach. Cases with a vanishing twin, confirmed trisomy 21, 13 or 18 in one or both twins, and incomplete pregnancy outcome data were excluded. The initial fetal fraction result was used for cases with an initial non-reportable result. Univariate analyses were performed using Pearson Chi-square, Fisher's exact and Kruskall-Wallis tests. Multivariate logistic regression models were used to account for confounding. A total of 1,771 samples were sent for cfDNA screening. 72 cases with a vanishing twin, 57 cases with one or both twins diagnosed with trisomy 21, 18 or 13, and 247 cases with inadequate follow up were excluded leaving a total of 1,395 cases for inclusion in the analysis. The median maternal age was 35 years, the median gestational age was 12 weeks, the majority of twins were dichorionic and 590 (42%) were conceived with in-vitro fertilization. Black race, higher body mass index (BMI), chronic hypertension (CHTN), pre-existing diabetes (DM), and treatment with low molecular weight heparin were all associated with fetal fraction <5th percentile. Older gestational age (GA) at cfDNA blood draw was associated with fetal fraction >95th percentile. Fetal fraction <5th percentile was associated with gestational diabetes. This remained significant after adjustment for race, BMI, GA at blood draw and CHTN (aOR 2.11; 95% CI 1.12-3.99). In the univariate analysis fetal fraction >95th percentile was associated with having one or both twins with a birth weight <5th percentile and <10th percentile. However, after adjustment for race, BMI, GA at blood draw, and CHTN, fetal fraction >95th percentile was only associated with increased odds of having one or both twins with a birth weight <5th percentile (aOR 2.05; 95% CI 1.20-3.49). Women with a twin gestation with a low fetal fraction are at increased risk of developing GDM while women with an elevated fetal fraction are at increased risk of having one or both twins with a birth weight <5th percentile.
