- W4223486985 abstract "Kaze et al1Kaze AD Santhanam P Erqou S et al.Cardiorespiratory fitness and atherosclerotic cardiovascular outcomes by levels of baseline-predicted cardiovascular risk: the Look AHEAD Study.Am J Med. 2021; 134 (e1): 769-776Google Scholar demonstrated the potential utility of assessing fitness in atherosclerotic cardiovascular disease (ASCVD) risk stratification in type 2 diabetes in the previously published article, entitled “Cardiorespiratory Fitness and Atherosclerotic Cardiovascular Outcomes by Levels of Baseline-Predicted Cardiovascular Risk: The Look AHEAD Study” in the current issue of The American Journal of Medicine. Showing the benefit of fitness in certain risk groups is essential because it can enable us to focus our limited resources on patients for whom cardiorespiratory fitness intervention would be most effective in this ageing society where ASCVD has been becoming a global burden.2Einarson TR Acs A Ludwig C Panton UH. Economic burden of cardiovascular disease in type 2 diabetes: a systematic review.Value Health. 2018; 21: 881-890Google Scholar Their robust analysis, which included multi-ethnic and multi-racial samples, also strengthened the external validity of the findings. However, this study seems to lack a clinical perspective. Ten-year ASCVD risk calculation is a convenient and comprehensive concept that can be used to share treatment goals and current status information with patients. Even with high risk of ASCVD, some patients do not clearly understand how dangerous their lifestyle can be. Shared decision-making is always crucial in any sort of medical care setting. Thus, educating patients with clear numbers can be effective. However, ultimately, management plans should properly be modified to individual cases. Some patients need blood pressure control, whereas others need to take lipid-lowering agents, which was not illustrated in the table, although the calculation results may be the same.3Goff Jr, DC Lloyd-Jones DM Bennett G et al.2013 ACC/AHA guideline on the assessment of cardiovascular risk: a report of the American College of Cardiology/American Heart Association Task Force on Practice Guidelines.J Am Coll Cardiol. 2014; 63: 2935-2959Google Scholar Is that important to estimate the future risk of ASCVD for certain patients? Yes, it can motivate patients to lead healthier lifestyles and to comply with the medications that they should keep taking. Then does this calculation alter our daily practice? We do not know yet. We should not judge the treatment directions solely by the risk calculation." @default.
