- W4224254898 abstract "The recent review by Bridoux et al., 1 Bridoux F. Leung N. Belmouaz M. et al. Management of acute kidney injury in symptomatic multiple myeloma. Kidney Int. 2021; 99: 570-580 Google Scholar in Kidney International, on the management of acute kidney injury in multiple myeloma states that pamidronate is preferred to zoledronic acid (ZA) for treating hypercalcemia in multiple myeloma due to lower risk of kidney complications. We do not believe this recommendation is supported by published data, including data cited by the authors. •Weide et al.2 Weide R. Koppler H. Antras L. et al. Renal toxicity in patients with multiple myeloma receiving zoledronic acid vs. ibandronate: a retrospective medical records review. J Cancer Res Ther. 2010; 6: 31-35 Google Scholar is a retrospective study of 84 hypercalcemic patients comparing ZA versus ibandronate; pamidronate was not studied. •The review by Perazella and Markowitz 3 Perazella M.A. Markowitz G.S. Bisphosphonate nephrotoxicity. Kidney Int. 2008; 74: 1385-1393 Google Scholar concludes that ZA is superior to pamidronate for correcting hypercalcemia and highlights the podocyte toxicity of pamidronate. Management of acute kidney injury in symptomatic multiple myelomaKidney InternationalVol. 99Issue 3PreviewSymptomatic multiple myeloma is commonly complicated by acute kidney injury through various mechanisms. The most frequent is the precipitation of monoclonal free light chains with uromodulin in the distal tubules, defining light chain cast nephropathy. Early diagnosis and identification of the cause of acute kidney injury are required for optimizing management and avoiding chronic kidney injury that strongly affects quality of life and patient survival. In light chain cast nephropathy, often manifesting with severe acute kidney injury, renal recovery requires urgent intervention based on vigorous rehydration, correction of precipitating factors, and efficient anti–plasma cell chemotherapy to rapidly reduce the secretion of nephrotoxic free light chains.
