TB is an infectious disease that attacks the lungs, which is caused by the bacterium Mycobacterium Tuberculosis. As a result of ineffective airway clearance, patients with accumulation of secretions cause difficulty in breathing which obstruct the fulfilment of oxygen supply in the body and causes cell death, hypoxemia and decreased consciousness so that it can lead to death if not treated. Therefore, assistance is needed to remove secretions so that airway clearance is effective again, with deep breathing techniques, effective coughing, chest physiotherapy, nebulizer, suction, and oxygen administration. The aims of this study is to implement nursing care in pulmonary tuberculosis patients with ineffective airway clearance problems at Renggiang Public Health Center. This research is a descriptive study in a case study design consisting of the stages of the nursing process: assessment, nursing diagnosis, nursing intervention, nursing implementation, and nursing evaluation of 2 patients with tuberculosis. The results of ineffective airway clearance can be resolved by observing vital sign within the normal range, adjusting the semi-Fowler or Fowler position, teaching effective coughing, monitoring breath sounds, giving warm drinks, monitoring respiration and status O2, collaboration with doctors in drug administration. At the beginning of the assessment, patient 1 complained of shortness of breath, coughing with phlegm, difficult to expel phlegm, orthopnea, rhonchi in the left and right lungs +|+, excess sputum became more comfortable, shortness of breath and reduced sputum production, coughed effectively, could expel phlegm. Meanwhile, patient 2 initially complained of shortness of breath, coughing with phlegm, difficulty in expelling phlegm, orthopnea, crackles in the left and right lungs +|+ became more comfortable, breathlessness was still present, sputum production was reduced, coughed effectively, could expel even a little phlegm. Providing nursing care can reduce the problem of ineffective airway clearance in Patient 1 and patient 2. Keywords: Nursing Care, Tuberculosis, Ineffective Airway Clearance ABSTRAK Tuberkulosis (TBC) masih merupakan masalah kesehatan yang menjadi tantangan global. Jumlah kasus tuberkulosis berdasar jenis kelamin pada laki-laki lebih tinggi dari pada perempuan yaitu 1,3 kali dibandingkan perempuan.TBC adalah penyakit menular yang menyerang paru-paru, yang disebabkan oleh kuman Mycobacterium Tuberculosis. Akibat bersihan jalan nafas tidak efektif penderita penumpukan sekret menyebabkan terjadi kesulitan bernapas yang menghambat pemenuhan suplai oksigen dalam tubuh serta membuat kematian sel, hipoksemia dan penurunan kesadaran sehingga dapat mengakibatkan kematian apabila tidak ditangani. Maka, perlu bantuan untuk mengeluarkan sekret sehingga bersihan jalan napas kembali efektif, dengan teknik nafas dalam, batuk efektif, fisioterapi dada, nebulizer, suction, dan pemberian oksigen. Penelitian ini bertujuan untuk mengetahui gambaran pelaksanaan tindakan keperawatan pada pasien tuberculosis paru dengan masalah bersihan jalan napas tidak efektif di wilayah kerja UPT Puskesmas Renggiang. Penelitian ini merupakan penelitian deskriptif dalam rancangan studi kasus yang terdiri dari tahapan proses keperawatan: pengkajian, diagnosa keperawatan, intervensi keperawatan, implementasi keperawatan, dan evaluasi keperawatan terhadap 2 pasien penderita tuberkulosis. Setelah dilakukan proses asuhan keperawatan terhadap 2 pasien diperoleh hasil bersihan jalan nafas tidak efektif dapat teratasi dengan observasi TTV dalam rentang batas normal, atur posisi semi fowler atau fowler, ajarkan batuk efektif, monitor bunyi napas, beri minuman hangat, monitor respirasi dan status O2, kolaborasi dengan dokter dalam pemberian obat. Saat awal pengkajian pasien 1 mengeluh sesak napas, batuk berdahak sulit untuk mengeluarkan dahak, ortopnea, suara ronchi di paru kiri dan kanan +|+, sputum berlebih menjadi merasa lebih nyaman, sesak dan produksi sputum berkurang, batuk secara efektif, bisa mengeluarkan dahak. Sedangkan pasien 2 awalnya mengeluh sesak napas, batuk berdahak sulit untuk mengeluarkan dahak, ortopnea, suara ronchi di paru kiri dan kanan +|+ menjadi merasa lebih nyaman, sesak masih ada, produksi sputum berkurang, batuk secara efektif, bisa mengeluarkan dahak walau sedikit. Setelah dilakukan tindakan keperawatan terhadap pasien 1 dan pasien 2 tuberculosis paru dengan pemberian asuhan keperawatan dapat mengurangi masalah bersihan jalan napas tidak efektif.
