Programs found it challenging to familiarize themselves with applicants through virtual venues sufficiently, and as a result, many deferred for a relatively less risky approach by accepting students from their home institution with whom they presumably had an ongoing relationship.2 Although programs consider applicants who rotate on their service both physically and virtually, physically present students would have a tremendous advantage as they would have come to know the faculty intimately through scrubbing with them in the operating room, accompanying them to clinic, and participating in all aspects of patient care—all aspects that a virtual rotation cannot provide. Therefore, students at a given medical school are more likely to be selected than those from other medical schools who only did a virtual away rotation. While confidence in future resident selection is of utmost importance, this trend, if it were to continue, would likely result in an overall less than optimal match result due to a reduction in considered candidates. It would also limit diversity among accepted applicants, as medical students from the same institution would more heavily populate residency programs. The permanent replacement of away rotations and interviews with virtual options requires acknowledgment and remediation of the disproportionate effect this would have on orphaned and international medical students.3 We suggest a further emphasis on program involvement in American Council of Academic Plastic Surgeons’ virtual webinars to ensure an opportunity for prospective applicants and programs to interact with one another. Furthermore, if interviews remain virtual, the Association of American Medical Colleges will need to consider limiting the number of interviews an applicant accepts to avoid further interview disparity.4 Lastly, creating opportunities to generate interest in plastic surgery earlier in medical education should be encouraged. It can allow for additional time to generate relationships in the field, interact with physicians from different programs, and explore those institutions that they feel most align with their passions.
