To determine the relationship between motivation and self-perception with risk factor secondary prevention behavior in coronary heart disease patients. This study is a descriptive correlation using a cross-sectional approach with a total sample of 62 patients with coronary heart disease. The sample collection method is purposive sampling. The research instrument consisted of a questionnaire about motivation, self-perception, and secondary prevention behavior of coronary heart disease risk factors. Data analysis in this study used the Spearman Rank test with a p-value of 0.000 <0.05. There is a relationship between motivation and prevention behavior (0.000 < 0.05) with an p-value of 0.509. And there is a relationship between self-perception and prevention behavior (0.000 < 0.05) with an p-value of 0.445. There is a relationship between motivation and self-perception with risk factor secondary prevention behavior in coronary heart disease patients. Diperlukan upaya pencegahan baik primer maupun sekunder untuk pengendalian faktor risiko penyakit jantung koroner tersebut. Tujuan penelitian untuk mengetahui hubungan motivasi dan persepsi diri dengan perilaku pencegahan sekunder faktor risiko pada pasien penyakit jantung koroner. Penelitian ini adalah descriptive correlation menggunakan pendekatan cross sectional dengan jumlah sampel responden 62 pasien penyakit jantung koroner. Metode pengumpulan sampel purposive sampling. Instrumen penelitian terdiri dari kuisioner tentang motivasi, persepsi diri dan perilaku pencegahan sekunder faktor risiko penyakit jantung koroner. Analisa data pada penelitian ini menggunakan Uji Spearman Rank dengan nilai p-value 0,000 < 0,05. Ada hubungan antara motivasi dengan perilaku pencegahan (0,000< 0,05) dengan nilai r hitung 0,509. Dan ada hubungan antara persepsi diri dengan perilaku pencegahan (0,000< 0,05) dengan nilai r hitung 0,445. Ada hubungan motivasi dan persepsi diri dengan perilaku pencegahan sekunder faktor risiko pada pasien penyakit jantung koroner.
