- W4225859714 abstract "Background: HRS is a severe complication in patients with advanced liver cirrhosis, and is associated with significant morbidity and mortality. HRS is treated with Terlipressin and Albumin, but only about 50 % patients respond to therapy. Aims: To assess predictors of response to therapy for HRS Methods: All patients with liver cirrhosis and acute kidney injury (AKI), who fulfilled the revised criteria of the International Ascites Club for the diagnosis of HRS were included. Patients with hypertension, diabetes mellitus, renovascular disease, CKD, pre-renal AKI, acute tubular necrosis, glomerulonephritis, urinary obstruction were excluded. Baseline demographic, clinical and laboratory parameters were recorded, and renal resistive index (RRI) was done by duplex doppler ultrasonography on day 0 and day 3. Patients were given standard of care for HRS. Results: 17 patients with HRS were included. The cohort had median age of 48 (25-70) years and 15 (88.2%) were males. 12 (70.5%) patients showed complete response to therapy, 5 patients did not. Only low platelet count was found to be a predictor of non response to therapy on univariate analysis, but was not significant on simple linear regression model. This is in contrast to previous studies which have shown that a high serum bilirubin and high serum creatinine were predictors of non response to HRS. RRI was higher in non response group both on day 0 (0.683 Vs 0.66; P= 0.394) and on day 3 (0.645 Vs 0.64; P=0.423). The small sample size is a limitation of this study. 28 day mortality was 3 (60%) in the no response group and 1(8.3%) in the response group, and this difference approached statistical significance. (P=0.053) Conclusion: None of the baseline parameters predicted a response to therapy in HRS. However this needs to be studied with a larger sample size. 28 day mortality was higher in non responders." @default.
