- W4226441014 abstract "To the Editor: I utterly agree with Yudkowsky and Szauter 1 about the impending losses incurred by the recent, unexpected suspension of the United States Medical Licensing Examination (USMLE) Step 2 Clinical Skills (CS). However, their vision of the expected gains from eliminating the exam remains vague, uncertain, and obscure. The last vestige of an attempt to highlight both patient centeredness and the quintessential value of clinical skills, the Step 2 CS is now being forsaken to simplify the process, contain costs, and please students. Patient-centered care is unanimously lauded, but in fact, ward rounds have often been moved from the bedside to the conference room, 2 and clinician-educator role models who actually speak to patients, skillfully examine them, and demonstrate support and shared decision making in front of trainees are becoming scarce. 3 Medical students will no doubt get the message—because communication and examination skills are not important enough to be tested as a graduation requirement, are they worth the time and effort needed to acquire them? Transferring the onus to medical schools can hardly compensate. Not only do schools vary widely in standards but beleaguered educators may soon be skipping more clinical core instruction encounters than the authors of “New opportunities” imagine. Standardization will be lost, and evaluation of international medical graduates is bound to be deficient. Consequently, the dwindling clinical skills and confidence of trainees will likely lead to an exponential increase in test ordering. In an era where medical graduates go on to become residents whose survival depends on computer time and multitasking at the expense of <10% patient face time, 4 one of the last bastions—demanding proof of basic clinical competencies—has now been removed. Unfortunately, I cannot share the authors’ optimism for the results of this change. When time is becoming more constrained and patients more complex, 5 tested ability of our medical graduates to efficiently elicit information, form a constructive plan, and simultaneously relate to “the patient with the disease” remains more important than ever." @default.
