- W4229035778 abstract "Percutaneous treatment of coronary artery disease has become a pillar of cardiology practice. As medical technology advances, interventionalists are treating patients with increasingly complex coronary anatomy. Consequently, potentially lethal complications occurring during percutaneous coronary intervention including coronary dissection, perforation, and the no-reflow phenomenon. As a result, interventional cardiologists need to be vigilant for these complications and initiate therapy without delay. The management of coronary dissection depends on the patency of distal vessel and extent of its propagation. In general, in the presence of acute myocardial infarction or abrupt closure, coronary stenting is mandated. Coronary artery dissection complicating PCI is rare, but it is associated with high morbidity and mortality; treatment strategy vary directly depends on hemodynamic status, type and location of the perforation. Hence, prompt recognition, immediate balloon tamponade, addressing hemodynamic instability, and specific treatment are mandatory. Coronary no-reflow occurs frequently during STEMI PCI. No-reflow is associated with larger infarct size, reduced ejection fraction, and higher mortality. To prevent no-reflow, long ischemic and door-to-balloon times, long stents, and high-pressure stent deployment should be avoided. When recognized, early pharmacological intervention proves beneficial." @default.
