There is no doubt that the availability of an intubating fibrescope and the ability to use it can make the management of most difficult airways more successful (and less stressful!). This book is aimed at the beginner in fibreoptic intubation. It includes a CD ROM, which has video clips on different aspects of fibreoptic intubation (such as holding the scope, fibreoptic views through the nose or mouth). The book discusses the history of fibreoptic intubation, the principles of fibreoptic light transmission, the indications for fibreoptic intubation, local anaesthesia of the airway, the conduct and complications of fibreoptic intubation and the care and maintenance of scopes. It ends with a look at teaching and four appendices including a bibliography, useful addresses and an index. It contains many photographs and pictures of good quality. The scopes are based on the Pentax group of instruments, the Company that sponsors this book. In view of the target audience, i.e. beginners, I would make the following comments. There are many fairly important statements both in the introduction and in the appendices, which are unreferenced and therefore difficult to ratify. I am surprised that in the technique section, the author does not mention how important it is to hold the fibrescope straight in order to control the tip and I would have thought that teaching with a camera would be almost mandatory – it is certainly easier! He also comments that teaching trainees to use the scope from the front of the patient is difficult. He does not make the obvious suggestion of swapping your hands around so the scope stays in the neutral position (i.e. with the thumb control at the head end and index finger manipulating the control lever). The author's technique of awake intubation is described in some detail, but splits the endoscopy and insertion of the tube in two different parts of the book (Awake intubation and Tricks, tips and techniques). It would have been better to describe in detail a technique which maximises success from start to finish and perhaps even demonstrating the technique on the CD ROM. Furthermore, the author uses droperidol as part of his sedation technique, a drug which when injected may produce a tranquil placid patient who later complains of unpleasant sensations of mental agitation and restlessness. In the local anaesthetic techniques, the author describes several methods of anaesthetising the airway, including glossopharyngeal nerve block and the use of Krause's forceps. I am not sure how commonly the latter two techniques are used, if at all. The author continually reminds the reader of how expensive and fragile the fibrescopes are and there is a good chapter on the maintenance of fibrescopes, a subject often ignored by those who use them. I have shown this book to fibrescope beginners and they have found some of the chapters and pictures useful. I will therefore be donating my copy to the registrar's library in the hope that they use it to improve their knowledge of fibreoptic intubation." @default.
