We did not study the duration and dosage of medications used to control inflammation and elevated pressure in the medical therapy group after 1 month, but intensive treatment was not the rule. If patients had persistently elevated pressure or inflammation, vitrectomy with removal of lens fragments was performed. While recurrent inflammation from retained lens particles after 1 year is possible, we did not note this in any of our patients, despite the relatively large size of the fragments. We believe the good outcome in this group speaks for itself. It is worth noting that in our study, vitrectomy and removal of retained lens fragments was performed in most patients with fragments. It is also worth noting that there is a subset of these patients in whom retained lens fragments are well tolerated. We look forward to future work in these patients, which will enable us to predict which patients will require early surgery and in which patients further surgery can be safely deferred." @default.
