- W4230170867 abstract "To characterize and compare the sonographic features of exophytic serous borderline ovarian tumours (ESBOT) with those of invasive serous ovarian carcinoma (ISOC). 7 patients with histological diagnosis of ESBOT and 10 cases with ISOC, both depicting an exophytic growth pattern, were identified retrospectively. All cases were evaluated in the O&G Ultrasound Unit, Bnai Zion Medical Centre between 2011 and 2019. The masses were characterised according to the IOTA terms. Demographic, clinical, sonographic, operative and pathological data were collected and analysed. Differences in age, personal or family history of malignancy, mean CA-125 blood levels, maximal diameter of the lesion, presence of ascites at the time of diagnosis and presence of peritoneal implants did not reach statistical significance. Patients with ESBOT were detected at earlier stages (I, II) compared to ISOC (p = 0.035). In all ESBOT patients a unilateral solid adnexal mass was demonstrated, wrapping an apparently normal ovary with regular borders. The mass contained tiny cystic inclusions and calcifications and a minute external irregularity on the interface of the mass against pelvic fluid. On colour Doppler study an intratumoural vascular bundle (ovarian vessels running along the IPL) with radial branching towards the exophytic tumoural tissue was observed, which correlated with exophytic tumour growth. These features could not be observed in any of the ISOC cases (p < 0.001). In 42.8% of the ESBOT cases, additional unilocular-solid components were detected, whereas all ISOC patients presented with multilocular-solid tumour morphology (p < 0.001). The interface of the external mass border with adjacent pelvic wall was regular in all ESBOT cases, but irregular in 80% of the ISOC patients, suggestive of an invasive tumour (P = 0.002). Exophytic serous borderline ovarian tumours can mimic invasive serous ovarian cancer. Our results suggest that ESBOT can be characterised by specific B-mode and colour Doppler features enabling differentiation from ISOC." @default.
