- W4231376306 abstract "Despite recent controversy regarding its usefulness in the evaluation of straightforward stress incontinence (SUI), urodynamic testing has been routinely advocated in patients with pelvic organ prolapse (POP) to assess bladder and urethral function before repair. As noted by the authors, occult SUI has been noted in up to 80% of patients with severe POP, and their approach, used by many, is to perform a midurethral sling (MUS) in patients with urodynamic SUI, even in the absence of symptoms. Others have cited the risk of voiding dysfunction, de novo overactive bladder symptoms, and operative and perioperative complications as reasons to avoid simultaneous MUS. Perhaps few would argue that urodynamically proven SUI should be treated at the time of POP repair; a more troublesome scenario is when symptomatic SUI cannot be demonstrated, or when no SUI is demonstrated or reported. “Inside-out” Transobturator Tension-free Vaginal Tape for Management of Occult Stress Urinary Incontinence in Women Undergoing Pelvic Organ Prolapse RepairUrologyVol. 76Issue 6PreviewTo assess the safety and effectiveness of the “inside-out” transobturator tension-free vaginal tape procedure for the management of occult stress urinary incontinence (SUI) in clinically continent women undergoing prolapse repair. Full-Text PDF ReplyUrologyVol. 76Issue 6PreviewThe management of occult stress urinary incontinence (SUI) is both controversial and challenging.1,2 By definition, patients with occult SUI are clinically dry. However, in modern medicine, we do not rely on symptoms only but also use various objective measurements. Previous studies have demonstrated that urodynamic evaluations can unmask occult SUI in ≤80% of clinically continent women with severe pelvic organ prolapse (POP). How many of these patients will benefit from a preventive anti-incontinence procedure is currently unknown. Full-Text PDF" @default.
