Early diagnosis and immediate management are essential to prevent sexual complications. The aim of this study was to determine clinical features of penile fracture, to evaluate early results of surgical management and to assess postoperative sexual satisfaction. METHODS Between January 1985 and December 2008, 143 patients were treated surgically for penile fracture. The mean age was 35 years (18 -73). Sixty-four patients were summoned and evaluated postoperatively by the international index of erectile function. RESULTS Diagnosis was done between 1 hour and 10 days after the distress. Most common causes of fracture were forced hand manipulation (48%) or trauma during sexual intercourse (20.3%). Patients typically present with complaints of a penile “cracking” sound (96%), sharp penile pain, rapid detumescence and swelling (100%). Penile haematoma was observed in 97%, five patients had urethral bleeding and voiding difficulties suggesting associated urethral lesion. Immediate surgical treatment was performed via a sub coronal circumferential incision (96%) of cases and a localized longitudinal penile incision (4%). Corpus cavernosum fracture was bilateral (5) and unilateral (118). Five urethral injuries were noted (2 complete section). Treatment consisted on repair of the corpus cavernosum underlying defect. Anastomosis of the transected urethra was performed over an urethral catheter after suprapubic cystostomy catheter placement. There were no significant immediate complications and most patients (94%) were discharged on the first postoperative day. Patients were followed up between 6 months and 14 years. Subjective good erectile function was reported in 97.6% of cases, pain during erection in 9% of cases and low penile curvature in 2% of cases. No subsequent sexual problems were observed. CONCLUSIONS Diagnosis of penile fracture is usually made clinically. Immediate surgical management has good results. It guarantees early return to satisfactory sexual activity and normal voiding.
