Оценить ближайшие и отдаленные результаты операций непрямой реваскуляризации у пациентов с критической ишемией нижних конечностей (КИНК) на фоне дистальной стеноокклюзии артерий.Материалы и методы. Проанализированы результаты стандартного лечения и применения различных видов непрямой реваскуляризации у 210 пациентов с КИНК на фоне дистальной стеноокклюзии артерий. Этиологическими факторами КИНК были облитерирующий атеросклероз (142 пациента) и облитерирующий тромбангиит (68 пациентов). Уровень поражения артерий: бедренно-подколенный сегмент – 32 (15,2%), подколенно-берцовый сегмент – 68 (32,8%), берцово-стопный сегмент – 62 (29,5%), стопа – 31 (14,8%), многоуровневое поражение – 17 (8,1%). Для установления диагноза и оценки результатов лечения прослежены изменения в клиническом статусе пациентов, проводили ультразвуковую доплерографию с ангиосканированием, мультиспиральную компютерно-томографическую ангиографию, реовазографию, определяли сатурацию кожи кислородом. Ближайшие результаты лечения оценивали по шкале Rutherford R.B. et al. (1997).Результаты. У 48 пациентов (контрольная группа) проводилось стандартное лечение. Для стимуляции регионарного кровообращения были произведены следующие виды операции непрямой реваскуляризации: реваскуляризирующая остеотрепанация (РОТ) у 42 пациентов, поясничная симпатэктомия (ПСЭ) у 51 пациента, ПСЭ + РОТ у 38 пациентов, реваскуляризирующая остеотрепанация с внутрикостномозговым лазерным облучением (РОТ с ВКЛО) у 31 пациента. У пациентов с КИНК на фоне дистальной стеноокклюзии артерий операции непрямой реваскуляризации улучшают ближайшие и отдаленные результаты хирургического лечения, позволяют сохранить опорную функцию конечности и продолжать активный образ жизни. При этом наилучшие результаты получены при РОТ с ВКЛО. Сосудистая хирургияВыводы. Корреляционно-статистическим анализом выявлена достоверная зависимость полученных результатов от метода реваскуляризации. Техническая несложность выполнения операции РОТ с ВКЛО создает предпосылки для внедрения в ежедневную практику хирургических отделений многопрофильных клиник. Purpose. To study the short and long-term results of indirect revascularization in patients with critical lower extremity ischemia (CLEI) during distal steno-occlusion of arteries.Materials and methods. The results of standard treatment and application of various types of indirect revascularization operations in 210 patients with critical lower extremity ischemia during distal steno-occlusion of arteries were studied. The etiological factor of CLEI was obliterating thrombangitis in 68 patients and obliterating atherosclerosis in 142 patients. Occlusion of the thigh- knee segment in 32 (15.2%) patients, knee-calf segment – in 68 (32.8%) patients, occlusion of the shin-paw segment – in 62 (29.5%) patients, occlusion of the arteries of the paw – in 31 (14.8%) patients, and multiple steno-occlusion of the arteries – in 17 (8.1%) patients were detected. Clinical, dopplerographic, rheovasographic, multispiral computed tomographic angiographic examinations were performed to diagnose and evaluate the results of treatment, and oxygen saturation of the skin was determined. Close results of treatment were assessed according to Rutherford RB et al. scale (1997).Results. 48 patients received standard treatment (control group). Due to the impossibility of reconstructive surgery, 48 patients underwent revascularizing osteotrepanation (ROT), 51 patients underwent lumbar sympathectomy (LSE), 33 patients underwent ROT + LSE surgery, and 31 patients underwent ROT with intraosteocerebral laser irradiation (IOLI). Indirect revascularization operations improve the short and long-term results of treatment of patients, maintain the supporting function of the extremity, create conditions for patients to lead an active lifestyle. In this case, a more effective result is obtained in the ROT operation with IOLR.Conclusions. The correlation-statistical analysis showed that short and long-term results depend on revascularization methods. Since ROT surgery with IOLI is not technically difficult, it can be used in surgical departments of highly specialized clinics." @default.
