The guidelines advocate normalisation of HIV testing, and recommend increased testing to reduce undiagnosed infection and the adverse consequences of late presentation. They recommend HIV testing in all high-risk populations and where patients present with a wide variety of clinical indicator diseases. There was a wide public consultation before their launch. Since then, the Department of Health has funded several pilot projects to assess the implementation of the guidelines in different health-care settings and awarded the Medical Foundation for Sexual Health and HIV a grant to produce a book to publicise them to general practitioners and hospital doctors. Wider acceptance and implementation of the guidelines is an important next step. We hope that their publication in Clinical Medicine later this year will contribute significantly to this process.
