The authors studied bone mineral density (BMD) in 15 newly diagnosed patients with Crohn's disease (CD) and 15 with ulcerative colitis (UC). Lumbar and forearm BMD's were measured by dual energy x-ray absorptiometry. Twenty-three patients had repeat measurements 1 year later. Twenty had received systemic steroids. At diagnosis the mean BMD was significantly lower in patients with CD than with UC. Variables such as inflammatory activity, disease localization, and physical activity did not account for the differences. Treatment with corticosteroids did not result in further bone loss in 1 year. Comment: Low bone mineral density has been shown to be present in patients with inflammatory bowel disease (Gut 1987;28:410-5), but the pathogenesis has been obscure. Intuitively, one might conjecture that the usual causal factors related to poor bone health such as disease activity, lack of exercise, relative malnutrition, and corticosteroid treatment would be operative in this population. One would perhaps also intuit that altered bone mineral density would be an issue both in patients with CD and UC. Yet Ghosh et al. have convincingly shown that at the time of diagnosis low BMD is present in patients with CD but not UC. By studying newly diagnosed patients (who had been ill for relatively short periods of time), the authors effectively eliminated secondary osteopenic influences such as treatment, inactivity, malabsorption, and the cachexia of inflammation. Patients with colonic disease had bone densities similar to those with small bowel involvement, indicating that malabsorption is unlikely to be a major factor in the development of osteopenia." @default.
