After I read it I had a clear idea of what it is and diagnosed it twice within the following few months. According to the authors, “…the story of prurigo pigmentosa transcends characterization of the disease itself. It is a tale of parochialism in medicine, of imprecision in terminology and how difficulties posed by a Babel in the language of dermatology and pathology impede comprehension, of limitations posed by substandard pictorial material published in journals, and of hazards posed by lack of proficiency in dermatopathology.” Add that to the problems that non-English speakers have in contributing to English language journals and the failure of English speaking readers to pay attention to non-English ones and it may not be surprising that it took more than 30 years for someone like Böer and Ackerman to come along and set things right. Joan Mones and A. Bernard Ackerman, New York, 2004, Ardor Scribendi, Ltd, 115 pages. $20.00. ISBN 1-89-335724-4. Enrico Fermi once said, “Before I came here I was confused about this subject. Having listened to your lecture I am still confused, but on a higher level.” That sentiment does not do quite justice to this remarkable monograph, but it does capture one sentiment about its general subject area that reading it allowed me to experience—namely relief that I may not be the only person befuddled by a bewildering profusion of terms along the lines of borderline, minimal deviation, atypical, dysplastic, and “malignant”—or even “metastasizing”—when applied to supposedly benign neoplasms. In prose recognizably and delightfully Ackermanian, at times ranging from rococo to Ollendorffian, with phrases adverbial trumping those adjectival, the authors take direct aim at this complex and frequently confusing state of affairs, at many of the concepts, and at some of the fuzzy thinking that has led to a nightmare both semantic and practical. Mones and Ackerman do not mince words, and their fearless capacity to recognize horsefeathers when they see them and say so is one of the delights of this monograph. Whether benign or malignant tumors always fall into completely clear categories, whether or not there are shades of gray in diagnosis histopathological or practice clinical, and whether it is fair to say that an acorn is necessarily or not necessarily the same thing as an oak tree, ∗The concept to which I refer is that although an acorn can develop into an oak tree, it is not a tree (and some acorns do not grow into trees). I do not know if there is an earlier source for it, but it most recently caught my eye in a similar argument about “precursor” lesions put forward by James Spencer, MD, in a discussion of whether or not actinic keratoses are squamous cell carcinomas.∗The concept to which I refer is that although an acorn can develop into an oak tree, it is not a tree (and some acorns do not grow into trees).
