We both agree that management of aniridic keratopathy should be individualized based on patient age and stage of the disease. Likewise, we agree that for stage V disease (total limbal stem cell deficiency plus significant deep stromal opacification, endothelial disease, or failed previous transplantation), Boston KPro is the preferred treatment option. However, we differ in our approach in patients with stage IV aniridic keratopathy (limbal stem cell deficiency involving the visual axis but without deep stromal opacity or endothelial disease). In particular, they prefer a Boston KPro, whereas we recommend that limbal stem cell transplantation (LSCT) be considered first. We would like to present the following rationale for our approach.
