- W4236690159 abstract "I appreciate the opportunity to respond to the Letter to the Editor regarding our article in the January 2018 issue of the Journal of the Academy of Nutrition and Dietetics titled “Effectiveness of the Malnutrition Quality Improvement Initiative on Practitioner Malnutrition Knowledge and Screening, Diagnosis, and Timeliness of Malnutrition-Related Care Provided to Older Adults Admitted to a Tertiary Care Facility: A Pilot Study”1Silver H.J. Pratt K.J. Bruno M. Lynch J. Mitchell K. McCauley S.M. Effectiveness of the Malnutrition Quality Improvement Initiative on Practitioner Malnutrition Knowledge and Screening, Diagnosis, and Timeliness of Malnutrition-Related Care Provided to Older Adults Admitted to a Tertiary Care Facility: A Pilot Study.J Acad Nutr Diet. 2018; 118: 101-109Abstract Full Text Full Text PDF PubMed Scopus (35) Google Scholar The correspondents highlighted the success and importance of this project in increasing health care professionals’ knowledge about malnutrition as well as the improvements that occurred in response to the 3-month intervention in medical diagnosis of malnutrition, and the documentation of malnutrition-related interventions and discharge care planning. The correspondents also raise an important concern inherent in many pilot projects, the lack of preliminary data that otherwise would enable a priori determination of the sample size needed to have the power to detect statistically significant changes in outcome measures. Like our study, pilot studies, pilot projects, or pilot experiments are most often considered an opportunity to acquire preliminary data that will enable evaluating components of study or experimental design such as feasibility, practicality, recruitment approach, retention, adherence, and testing of survey tools and instruments. Pilots are also utilized to obtain the data necessary to be able to determine sample sizes in the design of larger full-scale research interventions or experiments. Thus, we agree that other investigators could use the results from our pilot project to generate hypotheses and design interventions using the novel Malnutrition Quality Improvement Initiative tools to investigate the efficacy on hard clinical and cost-effectiveness outcomes, including hospital length of stay and 30-day readmission rate.2Leon A.C. The role and interpretation of pilot studies in clinical research.J Psychiatr Res. 2011; 45: 626-629Crossref PubMed Scopus (911) Google Scholar As expressed in our article and in the correspondents’ letter, malnutrition continues to be a challenging condition that poses a substantial health care burden that cannot be diminished without improvements in clinical detection, medical diagnosis, and treatment interventions appropriate for the type of malnutrition occurring. In the current health care environment, where quality and costs of care are critical considerations,3Tappenden K.A. Quatrara B. Parkhurst M.L. Malone A.M. Fanjiang G. Ziegler T.R.
