- W4236732154 abstract "2048 Failure of maternal plasma volume expansion during pregnancy is a consistent characteristic of preeclampsia. This is partially due to increased membrane permeability associated with the preeclamptic condition. During a normal, healthy pregnancy, exercise induced plasma volume shifts are greater in late gestation as maternal membrane permeability increases, even in the absence of hypertensive disorders. PURPOSE: We hypothesized that women who developed preeclampsia would show greater transient fluid and protein shifts out of the vascular spaces during a postpartum exercise challenge than women who had normal pregnancies. METHODS: Study participants were women (age = 32.0 ± 4.9 yr) who had either a normal pregnancy (n = 14) or developed preeclampsia (n = 18) during a pregnancy within the past three years. After a 30-min rest period, participants performed 5 min of exercise on a cycle ergometer at 50 watts. Resting blood volumes were estimated using dye dilution, and additional blood samples were obtained immediately prior to, and during the last minute of exercise. Participants' heart rates and blood pressures were measured at rest and during the cycling. Transient plasma fluid and protein shifts were calculated by comparing pre to post hematologic indices (e.g., hematocrit). RESULTS: Plasma volume shifts from pre- to post-exercise were significantly greater in the former preeclamptics (−13.6 ± 8.0%; 319 ± 111 ml) compared to control women (−8.0 ± 5.0%; 181 ± 97 ml) (P<0.03). This increased hemoconcentration in the preeclamptics was not accompanied by a greater loss of plasma protein during exercise. Despite the fact that heart rate did not differ between groups, mean arterial pressures were significantly greater (P<0.001) in the former preeclamptics during both rest (87 ± 8 vs 78 ± 4 mm Hg) and exercise (118 ± 9 vs 111 ± 11 mm Hg). CONCLUSION: The fact that former preeclamptics respond differently to an exercise challenge may help in our ability to determine who may develop a hypertensive disorder during pregnancy. Future studies should be designed to test women prior to pregnancy who have a high risk of developing preeclampsia.
