- W4239168148 abstract "INTRODUCTION: Microsurgical removal of glomus jugulare tumors is frequently associated with injury of the lower cranial nerves. To decrease the morbidity associated with management of these patients, stereotactic radiosurgery was performed as an alternative to surgical resection. METHODS: From 1990 to 2002, 35 patients (mean age, 59 yr) underwent radiosurgery as primary management (n = 16) or for recurrent glomus jugulare tumors (n = 19). The mean tumor volume was 13.0 cm3; the mean tumor margin dose was 14.7 Gy. No patient was lost; the mean follow-up was 48 months. RESULTS: Normal facial movement was more common for patients having primary radiosurgery compared with patients with recurrent tumors (88 versus 53%, P = 0.03). After radiosurgery, 12 tumors decreased in size and 22 remained stable (tumor growth control rate, 97%). One patient had tumor progression noted at 99 months and underwent repeat radiosurgery. Five patients (14%) improved after radiosurgery, and 25 remained neurologically unchanged. Five patients (14%) developed new deficits (hearing loss, n = 4; facial numbness and hearing loss, n = 1) after a single procedure. For 24 patients with testable hearing before radiosurgery, hearing preservation was achieved in 86 and 81% at 1 and 3 years, respectively. No patient developed a new lower cranial nerve deficit after single radiosurgery; the patient having repeat radiosurgery had a new vocal cord paralysis one year after his second procedure. CONCLUSION: Radiosurgery provided tumor control with a low risk of new cranial nerve injury in early follow-up (mean, 4 yr). Radiosurgery can be safely used to primarily manage patients with glomus jugulare tumors that do not have significant cervical extension or for patients with recurrent tumors in this location. Longer follow-up is required to completely understand the role that radio-surgery should play for this patient group." @default.
