This observation supports the clinical notion that psychosis of epilepsy often arises against a background of the dysphoric disorder of epilepsy. Thus it might be nosologically related to the affective disorders rather than to classical schizophrenia. Purpose of review Some 40 years ago, the temporal lobe hypothesis of schizophrenia was put forward, by authors like Slater, based on the observation that a schizophrenia-like psychosis was commonly observed in patients with temporal lobe epilepsy. Subsequently, a lot of research in schizophrenia has centred on this hypothesis. Psychosis of epilepsy, on the other hand, has attracted much less attention. The purpose of this paper is to review the recent literature on progress in the understanding of the pathophysiology of the psychosis of epilepsy, focusing on the role of the amygdala. Recent findings Amygdala and hippocampal volume loss seems to be a trait marker for schizophrenia, possibly reflecting the genetic loading. The amygdala has long been known to play an important role in emotional information processing. Intact amygdala functioning seems to be crucial for normal development. In animals, lesioning of the amygdala results in behavioural disturbances that become apparent only in later stages of life. In this respect they parallel the course of schizophrenia in humans. In contrast to schizophrenia, in psychosis of epilepsy the amygdala seems to be enlarged, resembling that of patients with different affective disorders." @default.
