- W4239678490 abstract "Perhaps more so than in any other field within neurology, our emphasis as movement disorder clinicians, given the visible effect of hypo- and hyper-kinetic movements in day-to-day function, has always been improving our patients' quality of life. However, while our intentions remain pure, in many ways, our own training and biases as clinician-scientists can also be the limiting factors in our delivery of optimal care for our patients. How so? Our medical indoctrination of sound scientific principles has taught us to be skeptical about reported interventions that have not been proven efficacious in randomized, placebo-controlled trials. Federal institutions and industry partners have spent significant resources, and millions of dollars, in executing clinical trials that are designed to meet the very high but restrictive scientific bar we have set. We have graded the levels of evidence according to their rigor, and have created Task Forces to reinforce the implementation of interventions with “Class I evidence” and to warn or discourage us of the adoption of therapeutics with weaker levels of evidence. As a consequence, high impact journals, including Parkinsonism and Related Disorders, have prioritized the publication of methodologically rigorous studies, thereby naturally and unintentionally marginalizing clinical observations, qualitative studies, allied health-driven interventions that are difficult, if not impossible, to test using traditional double-blinded methods. The scientific tradition that protects against false claims and ensures genuineness of results, unfortunately, also builds an impenetrable wall for ideas not molded within its framework. Yet, every now and then, an article comes along that reminds us of the limitations of this seemingly unshakable scientific structure we have created, adopted and considered the ‘gold standard’." @default.
