- W4240520006 abstract "This chapter describes the surgical procedure for transilial bone biopsy and the laboratory methods for processing and analyzing the specimens. Prior to the actual biopsy, the patient should avoid aspirin for at least 3 days and should have nothing by mouth for 4 h. Suitable specimens can be obtained by use of a trephine with inner diameter of no less than 7.5 mm. The needle should always be very sharp and manual operation of the needle should be carried out as it results in better control. In order to obtain an intact biopsy, it is vital to use gentle pressure to advance the trephine. This is particularly the case in patients whose osteoporofic skeletons are very fragile. Pain during and after the biopsy procedure is the most feared complication. However, if generous amounts of local anesthetic are placed periosteally on both sides of the ilium, the bone biopsy procedure rarely evokes pain. The specimens should be dehydrated and defatted slowly, carefully, and completely to achieve good quality embedding and avoid creating tissue artifacts. These specimens should be passed through graded alcohols and acetone, and de-gas to prevent the formation of bubbles during embedding. The biopsy can give important clues to the pathogenesis of a metabolic bone disease, and the mechanism of a treatment intervention. Further, the information can be obtained rather quickly, at least quickly in the context of clinical bone research." @default.
