- W4241034951 abstract "Studies on adjuvant post-operative stereotactic radiosurgery (poSRS) to the cavity of resected brain metastases suggest that larger tumors (>2.0- 3.0 cm) have higher rates of recurrence and radionecrosis (RN), with local control 61-72% (Brennan eat al 2014, Liang et al 2015). This study aims to assess the impact of permanent low-dose Ceasium-131 (Cs-131) brachytherapy on local control and RN rate in patients treated for large brain metastases. After IRB approval, a total of 54 patients were accrued onto a prospective trial between 2010 and 2015. 46 treated metastases > 2.0 cm in preoperative diameter were included in this analysis. Patients underwent surgical resection of their metastases with intra-operative placement of stranded Cs-131 seeds as permanent volume implants in the resection cavity. The primary endpoint of this study was local freedom from progression (FFP). Secondary endpoints included regional and distant FFP, overall survival (OS), and RN rate. The median follow-up period was 15.0 months (range, 0.6 – 51.9 months). Treated metastases had a median preoperative diameter of 3.0 cm (range, 2.0 – 6.8 cm). Local FFP for all 46 lesions was 100% at the time of this analysis. Regional recurrences were found in 3/46 (7%) lesions, yielding a 1-year region FFP of 89%. Distant recurrences were found in 19/46 (41%) patients yielding a 1-year distant FFP of 52%. The median OS was 15.1 months with a 1-year OS rate of 58%. No variables, including lesion size, were found to be significantly associated with rates of regional FFP, distant FFP, or OS in univariate or multivariate analyses. No cases of local recurrence or RN were found in this cohort. Intraoperative Cs-131 brachytherapy is a promising and effective adjuvant therapy for large brain metastases requiring neurosurgical intervention." @default.
