- W4243546418 abstract "Impaired awareness of cognitive decline, also known as anosognosia, is a common symptom in AD as well as those at risk for AD. The objective of this study is to further our understanding of the neural substrate underlying anosognosia in amnestic mild cognitive impairment (aMCI) and mild Alzheimer’s disease dementia (AD) by using both functional and structural imaging modalities. Thirty-two older adults with aMCI and 13 patients diagnosed with mild AD were included in this study. All participants underwent clinical evaluations including clinical dementia rating (CDR), comprehensive neuropsychological assessments, MRI, and 18-Fluoro-deoxyglucose positron emission tomography (FDG-PET) imaging. In addition, all the participants completed the anosognosia questionnaire for dementia, the geriatric depression scale (GDS), and the subjective memory complaint questionnaire. Presence of anosognosia was determined by calculating the discrepancy between degrees of symptoms observed by the informants versus those recognized by the patient themselves. FDG-PET and T1 weighted volumetric MRI images were analyzed on voxel-by-voxel basis using SPM12 for the difference of regional gray matter (GM) density and regional cerebral metabolism according to the presence of anosognosia controlling GDS and CDR sum-of-boxes scores. Subjects with anosognosia (A+) showed hypometabolism in the left frontal pole, and mild hypermetabolism in the posterior part of the brain (i.e., right middle temporal gyrus and left superior parietal lobule) compared to those without anosognosia (A-). Similarly, A+ group showed decreased GM density in the anterior part of the brain compared to A- group, while A- group showed lower GM density than A+ counterpart in the posterior part of the brain. Conjunction analysis revealed that the regions of hypometabolism and atrophy in A+ compared to A- do not overlap. Consistent with previous studies, anosognosia-associated hypometabolism in the left frontal pole was positively correlated with semantic fluency, whereas it was not correlated with any memory measures contrary to some previous reports. Anosognosia-related regional atrophy was not associated with any neuropsychological test performance." @default.
