Moreover, the generalizability of research findings may be impacted by variables that influence individuals' decisions regarding participation in research. Thus, a self-selection bias often occurs when recruiting participants. In order to elucidate factors that influence decisions to participate in pain research, the present study compared people who responded to advertisements for a “sensory testing study,” but did not enroll into protocol, to those who participated. In recruiting subjects for a pain study, demographic data were collected during a semi-structured telephone screening procedure used to enlist subjects. Participants were screened in order to ensure eligibility criteria were met, and that callers were willing and able to take part in the study. A total sample of 776 (565 women, 211 men) callers was generated through local advertisements. Reasons for non-participation were collapsed across the following categories: medical exclusions, logistical issues, protocol reservations, female-specific barriers, and other. Chi square analyses were conducted to characterize differences between enrolled and non-enrolled participants. Women comprised a significantly greater proportion of the sample than men (p < 0.001). Moreover, significant sex differences in enrollment rates emerged (p < 0.001). Specifically, of women who called to inquire about the research (565) the proportion that enrolled (14.5%) was substantially lower than the proportion of men who enrolled (34.5%). The reasons for non-participation also differed across sex (p < 0.001), with non-enrollment among women being more likely to result from logistic barriers (e.g. lack of time) and protocol issues (e.g. hesitant to receive drug) (48.2%) compared to men (39.9%). These findings suggest that enrollment into pain studies may be associated with a sex-dependent self-selection bias.
