- W4244680055 abstract "Cancer patients often experience pain during the terminal phases of their disease. The failure of physicians to consider pain management a priority has resulted in inadequate pain relief in up to 80% of cancer patients. The concept of comprehensive medical management supported by the hospice movement as well as governmental and professional pain societies has resulted in appropriate pain control in 90% of patients. Despite these efforts, it is estimated that 5% or more of cancer patients are refractory to this approach. The Agency for Health Care Policy (AHCPR) and National Cancer Care Network (NCCN) guidelines recommend the epidural or intrathecal (IT) infusion in those cases refractory to medical management. Patients with cancer may experience nociceptive, neuropathic or mixed type of pain. Although epidural infusion of opiates may be more suitable for nociceptive pain, the concomitant use of adjuvants like local anesthetics or clonidine may improve the outcome in patients with neuropathic pain. A trial of epidurally administered analgesics will help to evaluate analgesic response, toxicity, and side effects. The reported quality of analgesia with the use of neuraxial analgesia may be adequate-to-excellent in up to 70% of intractable pain patients." @default.
