Is this really the answer? Recent demonstrations of the concept were fraught with operational obstacles, fierce industry opposition at the national and local levels, litigation, and congressional hostility. The upshot: The demonstrations never fully materialized, which suggests that such tests may not be feasible or even desirable. Congress now may risk implementing broad Medicare reforms without much information on their effects.The papers in this section focus on the trials and tribulations of four recent Medicare competitive-bidding demonstrations for managed care (Medicare+Choice) in Baltimore, Denver, Kansas City, and Phoenix. Bryan Dowd, Robert Coulam, and Roger Feldman give a detailed history of the projects and their analysis of what went wrong. Len Nichols and Robert Reischauer, who served on the congressionally mandated oversight committee for the most recent demonstrations, provide an alternative analysis. Four Perspectives then present the viewpoints of policymakers who were involved in various ways: Karen Ignagni, who heads the American Association of Health Plans, a managed care industry group; Barbara Cooper and Bruce Vladeck, who respectively ran the Medicare demonstration and the Medicare program during the 1990s; and Rep. Bill Thomas, chair of the House Ways and Means Subcommittee on Health.
