- W4244984955 abstract "This chapter discusses the management of cerebral edema/intracranial pressure (ICP) in stroke. Large supratentorial infarcts are frequently associated with edema and shift of the intracranial contents. Highly elevated ICP can develop and lead to brain death. Such life threatening complications may occur in as many as 10% of all strokes. Once there is significant intracranial shift and elevated ICP the mortality is in excess of 50%. One early sign of clinical neurologic deterioration, decreased level of consciousness, is not always due to globally increased ICP with cerebral hypoperfusion. There are several prospective studies that measured ICP upon clinical deterioration after a large hemispheric infarct. Metabolic derangement, infection, seizures, and medication side-effects must be ruled out as a cause of neurologic deterioration with decreased level of consciousness after a large hemispheric infarct. ACT scan can assess the degree of edema and midline shift and rule out hemorrhage within the infarct as a cause of neurologic worsening. Standard medical means of decreasing ICP can sometimes be beneficial but can exacerbate tissue shifts causing worsening of the neurologic condition and even further increased ICP. Intracranial pressure monitoring is indicated to support and monitor available medical therapies in a rational and controlled fashion. In selected patients surgical decompression may be indicated to improve the chance of survival." @default.
