I would like to thank the Editors for putting their trust in my judgment. The first decade of my life as a doctor was in a world without liposuction. We were faced with difficult challenges then, having to indicate or not a vertical or circumferential thigh dermolipectomy on a 16-year-old young woman. We all knew then that these procedures represented a “gray area” in surgical decision making (“should I operate or not”), and that the benefits of such procedures were questionable. Many times, the resulting scar was not a good option to obtain a good-shaped leg in a young patient who would probably never be able to wear a bathing suit confidently in public. Yves Gérard Illouz created liposuction, the technique to aspirate fat for aesthetic purpose. When he first showed the technique in Brazil in November 1980, it was clear to those of us who saw his presentation in Fortaleza that many things were going to change with this technique. At the time, liposuction was attacked by old professors, who compared it to other similar techniques, which curetted fat using small incisions, with terrible complications. When Illouz introduced liposuction in the United States at the ASPS Hawaii Congress of 1982, the technique found respect within the international plastic surgery community. Surgeons soon found out that Illouz's results were reproducible, and the technique became popular. In 1985, Pierre Fournier modified liposuction and created liposculpture—the technique to aspirate and, if necessary, to inject fat with aesthetic purpose. Instead of the aspirator, he used syringes, creating the so-called liposculpture. As soon as these techniques became popular, modifications were made that improved and, in some cases, made the technique dangerous. In the first years, when surgeons were cautious and still learning its boundaries, liposuction produced great results. The problems began when some began testing the limits of the technique, not only by changing the instruments but by modifying the technique itself. Although many improvements were good, some of them brought many complications. Something had to change, limitations had to be established. It became obvious that the limit of extracting 3 L of fat per surgery was wrong, because it completely left aside the fact that people vary in weight. Today, several countries have passed regulations specifying where the procedure should be performed and what percentage of the patient's weight can be removed in one stage. The Medical Council in Brazil has passed a regulation stating that lipoplasty is a surgical procedure, and can be performed only by doctors with surgical training. This is a step in the right direction, to demand better training of surgeons performing the technique. The idea of producing this issue of Clinics in Plastic Surgery was to present some of the most influential ideas that have been introduced in lipoplasty by surgeons who have created or modified tactics, and who represent different facets in the field.
