- W4245692767 abstract "The authors of the article described their experience of adjuvant hydrodistension under epidural anesthesia for interstitial cystitis (IC). While there was no control or comparison group, such as conventional hydrodistension, the result was remarkable, with 70% of the patients experiencing effectiveness for more than 3 months. The authors addressed the original National Institute of Diabetes, and Digestive and Kidney Disease (NIDDK) criteria, which included bladder pain; pelvic pain symptoms; cystoscopic findings; and urodynamics, as established by IC experts in 1987. We are currently in the process of redefining the NIDDK criteria, as will be discussed . In recent years, it was possible to diagnose IC on the basis of symptoms involving frequency and urgency with or without pain. Although hydrodistension as a diagnostic tool is losing ground in the USA, this is not internationally approved as illustrated by the lack of consensus on this issue at the International Consultation on Interstitial Cystitis, Japan (ICICJ), held on 28–30 March 2003 (International Consultation on Interstitial Cystitis, Japan. International Journal of Urology; 10 [Suppl. 1]). We now have to confirm the status of cystoscopy, hydrodistension and urodynamics in the diagnosis of IC. Cystoscopic findings confer a high level of diagnostic confidence and have moderate correlation with disease severity and histology. A high percentage of biopsies is undertaken during the cystoscopy, while hydrodistension offers various therapeutic benefits. Furthermore, cystoscopy and hydrodistension continue to be seen as important tests in IC diagnosis. On the other hand, urodynamics are not an essential part of routine clinical diagnosis. However, it was considered that urodynamics may be included if symptoms do not improve, in the case of male patients, if there is incontinence, or in the case of pelvic floor dysfunction. Moreover, as the authors mentioned, we have to focus more attention on treatment using hydrodistension. Unfortunately, the authors have not as yet demonstrated the mechanism by which to improve IC symptoms using adjuvant hydrodistension. Nevertheless, these results are promising as an IC treatment option. I hope they will redesign a randomized, controlled trial." @default.
