Over 1.7 million asthmatics were included, 10% of whom received AIT. AIT was associated with a significant decrease in the probability of asthma severity worsening, particularly from GINA (global initiative for asthma) treatment step 1–3 (hazard ratio 0.87, 95% CI 0.8–0.95). This review from KirsiJarvinen’s group provides a helpful overview of the key knowns and unknowns within the current research landscape. Following extensive literature review the authors provide a balanced commentary on the outcomes from the most recent seminal studies which have assessed a multitude of potential environmental risk factors for eczema and food allergy/sensitisation including early emollient use, microbiome, breastfeeding, vitamin D, water hardness and immunotherapy. Genotyping was also performed to six FLG mutations. A significant association was observed between FLG mutations, Fel d1 sensitisation and cat exposure. Of interest, children with FLG mutations were who exposed to dogs in infancy were less likely to be sensitised to any allergen (OR 0.16; 95% CI 0.03–0.86; P = .03) which is consistent with other studies suggesting a protective effect of early dog exposure. In this article Ivan Cardona et al present findings from their prospective single-blinded randomised cross-over control trial, where children aged 7–11 year with atopic dermatitis received a fortnight of twice weekly or twice daily bathing in a cross-over design.
