Tumor spread to the CNS, manifest as either neoplastic meningitis or intraparenchymal metastases, is in part due to the pharmacologic sanctuary created by the blood-brain (BBB) and blood-cerebrospinal fluid (CSF) barriers. As a result, tumor cells within the CNS are protected from the cytotoxic effects of systemically administered chemotherapy. Leukemias and lymphomas remain the most common cancers with a predilection for leptomeningeal spread.
