Neil leaves the journal in excellent health and on behalf of all the editorial board members (past and present) who had the pleasure of working with him, I'd like to thank him for his help and support over the past years and to wish him every success in the future. It's at times like this (I mean by the time I get to the second paragraph) that I wonder about the ‘acting’ bit. Why did I say yes? It seemed like a good idea at the time. As he signed out in his last editorial, Neil suggested that ‘the quality of this page is set to improve’—I'm not so sure! Where does one look for inspiration? As we end the coldest winter for years, the cricket and rugby look set to disappoint and I'm facing appraisal when the ink seems barely dry from the previous round, where is the silver lining on the dark cloud of winter? It then occurred to me that winter brought the best skiing I've had for years, at least my (Welsh) wife is happy with the rugby and I've got a whole new subject to discuss at appraisal, as this job is vacant and I'm ‘acting’ until after I meet the CD. Maybe things are looking up. So what priceless gems are to be found within the latest edition of TOG? Well, there is a veritable feast to whet the appetite as we move into 2009, with something for everyone in this edition. To begin with we have an excellent review of the management of hirsutism. Rebecca Swingler and colleagues present a practical guide through this sensitive subject, outlining treatment options and reminding us all of the importance of managing patient expectations as well as the underlying physical problem. LARCs is a wonderful acronym and in my mind conjures up visions of 1970s TV cop shows; however, Charlotte Fleming reminds us of the advantages and benefits of long-acting reversible contraceptives (LARCs to the uninformed) and her article is superbly useful to all those, no matter what discipline, who give contraceptive advice. I, for one, will be recommending it (not the LARC) to my physician colleagues who see women in their clinics prior to pregnancy. I can still remember as a trainee being told that ‘one day we will develop a vaccine to reduce cervical cancer rates…’ and here we are already with an update on the programme from Alison Fiander. An excellent review and I'm just trying to remember what else we may achieve amongst what they told me all those years ago. Bridging the clinical gap between obstetrics and gynaecology, Fowler and colleagues describe the process of establishing a perineal clinic. Developing this service as a link between birth trauma and urogynaecology may seem to many an obvious thing to do but the team highlight the clinical advantages as well as the patient satisfaction that their service has brought. In these times, when doctors’ hours are restricted and time is short, this service is truly multidisciplinary, with specialist midwives leading on service delivery. At first glance the obstetric articles in this edition may appear to be written for the fetal medicine subspecialist—well, look again. Ketan Gajjar and Chris Spencer deal with the subject of non-anti-D red cell antibodies in pregnancy and the subspecialist or generalist will be sure to benefit from their comprehensive review. We also have not one, but two articles on fetal infection. I know from my own practice that this issue presents to community and hospital practitioners alike. We have an overview from Meekai To and colleagues on fetal infection and an article specific to the management of primary and secondary cytomegalovirus infection from Fergus McCarthy and his fellow authors. Common problems comprehensively dealt with. As tempers shorten in outpatient waiting rooms and emergency admissions join a variable queue waiting to be seen in the overworked casualty department, Bryony Strachan and Jo Baden Fuller examine the issue of conflict and aggression in the modern NHS. Clearly an article to be read before the next directorate consultant meeting! We also have a view from alongside the profession on the issue of consent. Bertie Leigh presents us with a commentary on the evolving nature of the doctor's obligation towards the patient. I look forward to the correspondence and debate that I'm sure will follow in the letters pages. And finally, well I for one certainly hope not, we have our usual offering from James Drife. Despite retiring from clinical practice to take up the pen full time (surely not), he will, I hope, continue to grace our pages with his thoughts, comments and views for the foreseeable future at least." @default.
