- W4247233611 abstract "Guzzo et al 1 Guzzo T.J. Magheli A. Bivalacqu T.J. et al. Pathological upstaging during radical cystectomy is associated with worse recurrence-free survival in patients with bacillus Calmette-Guerin-refractory bladder cancer. Urology. 2009; 74: 1276-1280 Abstract Full Text Full Text PDF PubMed Scopus (37) Google Scholar found that patients with high-grade T1 bladder cancers or carcinoma in situ (CIS) often (42% and 38%) were upstaged (pT2) at cystectomy, and they had a worse recurrence-free survival than correctly staged patients operated for de novo muscle-invasive disease. The authors concluded that inaccurate staging, treatment delay such as receiving BCG therapy, or both may have contributed to worse outcomes. It is likely that inadequate staging was the primary culprit, as unfavorable biology alone cannot account for such high understaging rates. Pathological Upstaging During Radical Cystectomy Is Associated With Worse Recurrence-free Survival in Patients With Bacillus Calmette-Guerin-refractory Bladder CancerUrologyVol. 74Issue 6PreviewTo compare the outcomes of patients who were upstaged to pT2 at the time of radical cystectomy (RC) to those who were correctly staged as T2 before RC. Full-Text PDF ReplyUrologyVol. 74Issue 6PreviewClinical understaging of patients with a diagnosis of non–muscle-invasive bladder cancer is not uncommon and has been reported in several large series (references 4-9 of manuscript). Dr. Herr has appropriately indicated a variety of important factors (multiple tumors, completeness of resection, imaging studies, etc.) that should be considered regarding patients with high-grade non–muscle-invasive bladder cancer—all of which are routinely used at our institution when formulating treatment decisions in this patient population. Full-Text PDF Editorial CommentUrologyVol. 74Issue 6PreviewThis retrospective review compares the outcomes of patients with muscle invasive bladder cancer (MIBC), known before radical surgery (cT2 = pT2), with those of clinically non-MIBC, which appeared upstaged to MIBC after cystectomy. The latter group, especially if previously treated with BCG, had a significantly worse RFS (P = .04 and P = .03 after BCG), possibly because of understaging and subsequent delay in treatment.
