- W4247567609 abstract "At our institution, we have recently used more neuraxial and thoracic paravertebral anesthetic techniques and have become aware of common local practices in positioning that are not routinely illustrated in texts. First, transport gurneys can be used to facilitate patient positioning for regional anesthesia. As seen in Figure 1, the back of the gurney can be easily placed at a 90° angle to the foot. With the head of the gurney so elevated, it serves as a vertical platform thereby supporting the patient and preventing him from “slumping” from side to side during block placement.Figure 1: Patient positioning for regional anesthesia on a transport gurney. For placement of a lumbar neuraxial block in the sitting position, we make every effort to reduce the patient’s lumbar lordosis by elevating the knees relative to the hips. Figure 2 depicts a simple aid in accomplishing this end: we carefully tilt or “airplane” the table 30° toward the back once the patient has assumed the seated position.Figure 2: Patient positioning for a lumbar neuraxial block with the operating table “airplaned.”Sylvia Dolinski MD J.
