To the Editor: We read with interest the study of Starlinger et al.1 evaluating the ability of microRNA signatures to predict postoperative liver dysfunction in patients with liver cancer. The results showed that it could effectively predict postoperative liver dysfunction. After reading the article carefully, we have some suggestions to point out. First, their conclusions showed that preoperative microRNA signatures as a single indicator can effectively predict postoperative liver dysfunction.1 However, there are many factors affecting liver function after hepatectomy, including preoperative, intraoperative, and postoperative factors.2 A single preoperative indicator may be difficult to fully and effectively project postoperative liver function, especially for patients with cirrhosis. We believe that microRNA signatures can be used as a good indicator for evaluation of preoperative liver function and prediction of the patient's tolerance to maximum hepatectomy volume. Second, in the multivariate analysis of their study, patients were divided into major and minor groups only on the basis of hepatectomy volume in three segments, and future liver volume was used in Predictive Value of Preoperative Liver Function Tests. Liver volume is an important factor affecting postoperative liver function recovery.3 Standard residual liver volume (SRLV) and future liver remnant function (FLRV) can effectively predict the occurrence of postoperative liver dysfunction,4 whereas liver resection volume and future liver volume are not effective. We believe that the results of this study may be more interesting if the study includes SRLV or FLRV. Generally speaking, if combined with intraoperative and postoperative indicators, the results of this study would be more interesting and credible." @default.
