- W4248093352 abstract "To the Editor—Two recent commentaries, by Aly1 and de Klerk and de Klerk,2 review the role of continuous positive airway pressure (CPAP) as an alternative to mechanical ventilation in providing respiratory support for very low birth weight (VLBW) (birth weight <1500 g) and extremely low birth weight (ELBW) (birth weight <1000 g) infants. Both commentaries refer to several observational (nonrandomized) studies that suggest that CPAP, delivered by various means including nasal prongs, might have short-term advantages over mechanical ventilation, particularly in reducing the rate of chronic lung disease. Interestingly, both commentaries call for randomized, controlled trials to support or refute the hypotheses generated by the nonrandomized studies. The de Klerks2 also comment on the lack of scientific data concerning the long-term outcome for infants treated with CPAP, and they suggest that any trial should incorporate long-term outcome as one of the main endpoints. There are observational data suggesting that CPAP might reduce long-term neurosensory impairments. The most recent such study reported that the rate of cerebral palsy in infants <31 weeks’ gestational age fell significantly over time with a reduction in mechanical ventilation within eastern Denmark.3 Correctly, the authors of this nonrandomized study could not conclude that the reduction of mechanical ventilation was responsible for the fall in cerebral palsy, and they also suggested the need for a randomized, controlled trial.We have recently embarked on a randomized, controlled trial of nasal CPAP compared with intubation and mechanical ventilation in infants of 25 to 28 weeks’ gestation, who are breathing by 5 minutes of age, but who have respiratory distress. The major outcome for the trial is the incidence of survival and chronic lung disease at 36 weeks’ gestation. In addition, we will follow the health and development of survivors in early childhood. The Australian National Health and Medical Research Council have funded this multicenter study, and we have already recruited 65 infants out of a sample size of 600. This sample size will enable us to detect a clinically important reduction in rates of death or chronic lung disease from 30% to 20%.
