- W4248373372 abstract "Prostate cancer (PCa) is one of the most commonly reported cancers in families. In the current study, the authors assessed the role of family history (FH) in risk of a positive prostate biopsy. A total of 4360 men were retrospectively evaluated. They found that FH is an independent risk factor for PCa (odds ratio [OR] 1.6, P = .0001). Moreover, FH was significantly associated with high-risk PCa (P = .02). The existing literature regarding this topic is conflicting. Although FH was associated with increased risk of PCa in several studies, 1 Noe M. Schroy P. Demierre M.F. et al. Increased cancer risk for individuals with a family history of prostate cancer, colorectal cancer, and melanoma and their associated screening recommendations and practices. Cancer Causes Control. 2008; 19: 1-12 Crossref PubMed Scopus (20) Google Scholar the recently published REDUCE trial showed no significant difference between patients with and without FH. 2 Thomas 2nd, J.A. Gerber L. Moreira D.M. et al. Prostate cancer risk in men with prostate and breast cancer family history: results from the REDUCE study (R1). J Intern Med. 2012; 272: 85-92 Crossref PubMed Scopus (25) Google Scholar The current study confirms on a large cohort that FH is linked to an increased risk of PCa, especially with high-risk pattern. There are 2 limitations of this study. First, the authors did not distinguish between primary and second degree relatives. This is an important point as it might influence clinical management of FH positive patients. Second, the article does not provide any recommendation for PCa screening in case of FH. Nevertheless, the authors provide a thoughtful analysis of a relevant clinical issue. In the future, more insight in the exact gene or mechanism of transmission and further examining the role of epidemiological forms will help to improve PCa risk assessment in patients with FH. Does Positive Family History of Prostate Cancer Increase the Risk of Prostate Cancer on Initial Prostate Biopsy?UrologyVol. 81Issue 4PreviewTo assess the role of family history (FH) in the risk of a positive prostate biopsy (PBx) in a large North American biopsy population as earlier reports showed increased risk of prostate cancer (PCa) in men with a FH, but the risk has been limited to low grade prostate cancer in smaller studies, and the REDUCE trial found no such risk in North American patients. Full-Text PDF ReplyUrologyVol. 81Issue 4PreviewNotwithstanding all the “noise” surrounding recommendations for prostate cancer screening, our data do seem to clarify that North American men with a positive family history of prostate cancer truly are at increased risk of finding prostate cancer if biopsy is undertaken. Nevertheless, we also reaffirm our belief in the American Urological Association recommendation to establish a baseline prostate-specific antigen (PSA) level sometime around the age of 40 for all healthy men regardless of race or family history.
